Lots of innovative work is happening in South Gloucestershire to help improve the integration of health, care and the voluntary, community and social enterprise sector.
Come along on Thursday 2nd March from 12.30pm to hear more and learn from each other.
The event will be at: St Michael’s Centre, North Road, Stoke Gifford, Bristol BS34 8PD (near Bristol Parkway station and motorway links). A light lunch will be available.
We will take a close look at:
- Priorities within the emerging Bristol, North Somerset & South Gloucestershire STP
- What some organisations are doing to create a shift towards a more holistic system for health and care
- The Building Health Partnerships programme in South Gloucestershire, a cross sector initiative
- Sharing learning about cross-sector collaboration and partnership working
- Strengthening relationships and practice
- Exploring future opportunities.
Speakers will include Nancy Towers and James Butler of Social Enterprise UK and presentations on:
- Personalised Integrated Care (Age UK’s service for older people with long term conditions)
- Off the Record (wellbeing counselling for young people)
- Real Life with Dementia (a Council and AWP initiative to give carers tools they need when things get difficult)
- Community Connectors (Curo project to enable adults of all ages to live well)
